Prior to your surgery we will contact your insurance company for eligibility and benefit information. We will inform you of your estimated out of pocket expenses as quoted from your carrier.
We suggest that you contact your insurance company with any questions you may have regarding coverage.
After surgery, we will file your insurance claim for you. We will ask you to sign an “assignment of benefits” form so that the reimbursement may be sent directly to Crow Valley Surgery Center. |
